Dissects & Transects in One Pass

Remote Endarterectomy (RE) is a hybrid procedure, combining both surgical and endovascular techniques. LeMaitre Vascular’s suite of devices assists in the revascularization of long segment occlusions in the iliac artery, superficial femoral artery, and proximal popliteal artery.

The goal of RE is to use and maintain the native artery for as long as possible, postponing the need for bypass and restoring the option for future endovascular treatments. RE also helps to open and maintain large arterial side branches and collaterals upon removal of the atheromatous core. Patient benefits include preservation of the saphenous vein for future use. Decreased incisions allow for faster healing and fewer days spent in the hospital.

EndoRE Applications
  • Long Segment Occlusions: The EndoRE procedure is performed to remove long segment flush occlusions of the SFA and iliac arteries.
  • Stent Removal: Dissection is made around the occluded segment and the atheroma core inclusive of the SFA is removed.
  • Failed Fem-Pop bypass: The native SFA is recannalized after the fem-pop bypass graft becomes non-functioning.

EndoRE Devices

The MultiTASC® Dissection/Transection Device is designed to dissect and transect the plaque core at the designated endpoint with one pass, reducing the procedure time and potential for complications.

The MollRing Cutter® Transection Device is a tool designed to transect and remove the plaque core at the designated endpoint or site of the reconstitution of the artery. MollRing Cutters provide the ability to cut the core of the wall of the artery without tearing. The rounded bottom edge of the cutter ring allows for a smooth advancement from the proximal arteriotomy to the designated endpoint.

The Martin™ Dissector is a spatula-shaped dissector used to separate the plaque core from the wall of the artery. The dissector is designed to address moderate to severe calcification. The blunt dissecting edges of the spatula provide a more focused force when encountering calcification.

The Periscope™ Dissector is used when crossing the endpoint with a guidewire becomes difficult. It facilitates dissection through the closed endpoint. A nitinol needle is extended from the spatula’s palm into the true lumen of the artery. A .018” guidewire is advanced through the needle. Once in place, the Periscope Dissector is removed, leaving the guidewire in the distal lumen to assist with endovascular treatment of the intimal flap.

The EndoHelix™ Retrieval Device is a tool designed to remove fractured core segments from the lumen of the artery. The spatula end allows dissection of the fractured core from the wall of the artery. The helical wire is then advanced and tightened over the core, trapping the core segment for removal.

The Vollmar Ring Dissector provides a fixed-diameter circumferential dissection plane. The rings are most effective when dissecting soft, formed atheroma core with mild to moderate calcification.
